Organization Description:
This service provides specialized assessments, treatment, case management, and community support.

The inclusion criteria is 65+ years and psychiatric disorder must be present. A psychiatric disorder can be defined as a mental health condition that places health and safety and/or functional ability of an individual or others at risk if not treated i.e. psychosis, bipolar mood disorder, depression, anxiety, debilitating stress, complex trauma, extensive affect and/or behavioral dysregulation and/or suicidal or self-harm ideation.

One of the following may also be present:
· Requires community outreach.
· Allegations of neglect or abuse.
· Cognitive impairment.
· Diagnosed Dementia (may be under 65 years)

It is worth noting that Seniors Mental Health is not an emergency service, therefore seniors in psychiatric crisis will be referred to the Community Crisis Response Team.
